As a student at 糖心Vlog, you are required to complete these two steps prior to joining our campus community.

Tuberculosis (TB) Screening Questionnaire

All students must complete the Tuberculosis Screening Questionnaire online. Access to the form becomes available upon your acceptance to the University.

The TB Screening Questionnaire is located online in your portal.

Immunization Requirements

All new students (first-year, transfers, and others) are required by the University System of Georgia to have certain immunizations before starting school at Georgia Southern.

What’s Next?

Submit Your Insurance Card

Health Services is in-network with most major insurance plans as listed below. We will file a claim with any insurance plan. If you are uninsured or you do not want us to file your insurance plan, you will be responsible for paying the self-pay rates.

Required Forms & Acknowledgements

Health Services requires several forms and acknowledgements for patients attempting to access our services, including authorizations to treat and specific treatment enrollments.
